Your First Visit

Patient Forms

Upon entering our office, we will welcome you as a member of our family.  You will begin by filling out demographic and medical information on our state of the art KIOSK. You will answer questions regarding your health history and information on your current condition. Please bring information on medications (prescription, over-the-counter, and any supplements you take). If your visit is in relation to a motor vehicle collision or workers comp, we will need your insurance information and/or the adjustor who is handling the claim, as well as the claim number. Please bring photo ID, insurance information, and proof of military service or student enrollment, if applicable.  We offer a time of service courtesy for our patients; please contact us for additional details.


Vitals

Vital signs are a critical indicator of your overall health and any potential conditions. Vital signs are used throughout the medical industry and are considered a basic and necessary component of any new patient exam. These include obtaining an accurate height, weight, blood pressure, pulse rate and temperature. At Pacific West Chiropractic we use an advanced Welch Allyn vital sign machine connected to a Health-O-Meter professional scale to obtain consistent and accurate readings of your vital signs.


Examination

After getting a set of vitals, Dr. Katie Sill will perform a complete chiropractic examination testing your reflexes and flexibility. Other standard neurological, orthopedic, postural, and physical tests will be performed as well. However, nothing  will be done in our office  without your consent.


Imaging and Lab

Depending on your specific condition, we may recommend imaging or lab work. We are partnered with EPIC Imaging, Portland's leading outpatient imaging center, and with Quest Diagnostics and Tuality Lab Services. We can also provide basic urinalysis testing in clinic.


Report of Findings

Once the information is collected  and examinations are performed, Dr. Katie Sill will discuss with you findings and answer any questions including:

  1. Explanation of condition
  2. Treatment options and alternatives
  3. Treatment plan, including frequency, length and any referrals necessary
  4. Expected outcomes of care

After reviewing your health history,  goals, and results of your examination, Dr. Katie Sill will discuss recommendations and notify you if  your condition requires care with other providers.   Dr. Katie Sill  will  provide the best  treatment and  wellness program for your needs.


Treatment

At the conclusion of this initial appointment, you can choose to experience your first treatment. This may include spinal adjustments, physiotherapy (such as electric stimulation, therapeutic ultrasound, cold laser, sport taping), physical therapy rehab exercises and/or soft tissue manual or instrument assisted therapy.  We aim to assist and encourage our patients to take an active role in their health. Treatment is interactive, so you can express concerns about the different styles of treatment anytime.


Wellness Program

Prior to leaving, the doctor will suggest a  wellness program  to incorporate outside of treatment.  If you are in pain when you first come into our office this may include: ice or heat application instructions, certain activities or positions to avoid, and at home exercises and/or stretches.  In addition, our physician will occasionally recommend topical analgesics such as Cryoderm, Sacred Earth, or Biofreeze, or supplements from Biotics Research NW. If you desire,  our wellness team will work with you also to create healthy habits and routines for your lifestyle.  Every person is unique, therefore everyone requires a  customized wellness plan.  The purpose  of our wellness program is for   you  to achieve good spinal alignment, have a  healthy diet, exercise, and maintain a positive mental state.
